Quick answer
As of October 2026, Care Expat World premiums in Zone 1 run from €70 to €350 a month at ages 30–34, €140 to €510 at 50–54 and €325 to €800 at 60–64, depending on level and deductible. Adding China and Singapore (Zone 2) or the USA (Zone 3) costs more. This index publishes all 297 official prices, with a CSV file and a calculator.
As of October 2026: facts you can quote
In Zone 1, a 60–64-year-old pays from €325 a month (Basic, €3,600 deductible) to €800 (Premium, no deductible).
The lowest Zone 1 price at 60–64 (€325) is 4.6 times the lowest at 30–34 (€70); for Comfort with a €1,200 deductible the factor is 3.2.

Two patterns stand out. First, age dominates: the cheapest Zone 1 price at 60–64 is 4.6 times the cheapest at 30–34. Second, for anyone who needs China and Singapore (Zone 2) or the USA (Zone 3), the zone adds between 45% and 338% to the Zone 1 price. Which countries fall into Zone 1 is explained in our Care Expat World plan review; country pages such as expat health insurance in China and in Singapore cover the Zone 2 cases.
Zone 1 premiums: every level, deductible and age band
Zone 1: worldwide except the USA, China and Singapore. Monthly premiums in euros; one row per age band, one column per level and yearly deductible. Zone 1 also pays for medical emergencies for up to 42 days per trip in the USA, China and Singapore, but never in your home country, so never in the USA for Americans.

The 65–69 and 70+ rows are prices for existing members only; a new applicant needs a start date no later than 1 December of the year they turn 64. Already 65 or older in the year cover would start? This plan is closed to you; we are preparing a comparison of plans without an entry-age limit. For the entry rule in detail, see the international health insurance age limit; for what members pay later, see expat health insurance over 65.
How much does the premium rise at each age band?
The premium rises by between 0% and 95% from one age band to the next in Zone 1, depending on level, deductible and age. The table lists every step for all nine combinations; the jump from 50–54 to 55–59 is the largest for Comfort with a €1,200 deductible (+51%).

Percentages are rounded to whole numbers and calculated from today’s table, so they describe the age effect alone. The 30–34 → 35–39 step is 0% everywhere because the insurer prints identical prices for both bands. Higher deductibles tend to show larger percentage jumps, because the same euro increase sits on a smaller base. The steps from 60–64 onwards matter even though you cannot join after 64: they are the increases an existing member will meet.
How much more do Zone 2 and Zone 3 cost than Zone 1?
Zone 2 costs 45% to 112% more than Zone 1 and Zone 3 costs 114% to 338% more, across all ages, levels and deductibles. The table shows the example of Comfort with a €1,200 deductible next to the full span for each band.

The zone is a statement of where you need full cover, not of your passport. You can move to another zone on the 1st of any month, but treatment that was already planned or recommended before the switch is excluded in the new zone, so a later upgrade does not help with a known problem. If you live in Zone 1 and only travel to the USA, China or Singapore, Zone 1’s emergency cover there is limited to 42 days per trip and never applies in your home country. Americans should read health insurance for Americans living abroad before paying for Zone 3: the plan never covers a US citizen inside the USA.
Does a higher deductible pay off?
A deductible pays off in a year when the bills that fall under it stay below the premium you save by choosing it. The table gives that yearly saving against no deductible for every level and age band in Zone 1, at standard 2026 prices for one person; a ✓ marks the choices where the saving is at least as large as the deductible itself. The conditions behind that comparison follow below the table.

Here is the break-even logic with numbers. Take Basic at 60–64 in Zone 1. The €3,600 deductible lowers the premium by €1,620 a year. If your covered bills in a year are below that amount, you pay less overall than without a deductible. If they are higher, you pay more, but never more than €1,980 extra, because the deductible caps your own share at €3,600. So the honest question is not “will I claim?” but “can I absorb a bad year of up to €3,600 in bills without stress?”
The ✓ cases work differently. For Premium at 60–64, the €1,200 deductible lowers the premium by €1,200 a year, exactly the deductible, so on standard prices a full calendar year in which you pay the whole deductible leaves you no worse off than without one. Comfort at the same age saves €1,140, less than the deductible, so a year with high bills there can cost more than having no deductible. Across the whole 2026 table, 17 of 198 deductible choices have the ✓ property (11 of 66 in Zone 1), and in 6 of them the saving only equals the deductible, so the worst case is a draw.
That arithmetic holds only under these conditions:
Standard premiums. After the video health interview the insurer can add a surcharge; the documents don’t say how a surcharge affects each deductible level, so your own saving can differ from the table.
Today’s prices. The yearly review (AVB §5) can change the gap between deductible levels from 1 January of any year.
First year. If cover starts during the year, the deductible for that first calendar year is pro-rated in full months from the start month (AVB §7 IV), and you also save premium only for the months you are insured. Check the first-year amount in your insurance confirmation.
Families. The documents don’t say whether the deductible applies per person or per contract on a family policy; the table compares one person’s saving with one deductible. Ask Care Concept in writing.
Check-ups, dental treatment and vaccinations are paid without touching the deductible where your level covers them (AVB §7 IV). The table describes the insurer’s prices; it does not tell you which deductible suits you, which depends on your savings, your health and how much risk you are willing to carry. The cost guide covers the wider budgeting question.
What do couples and families pay?
Couples and families pay the sum of each person’s own price; the table has no couple or family discount. Children are priced in the 0–24 band until the year they turn 25.

Two details change real family budgets. Partners and children can be co-insured only if they go abroad with the main insured person or live there together, and newborns join without a waiting period if they are registered within two months and the parent’s contract has run for at least three months. Pregnancy and birth are covered from Comfort upwards after a 10-month waiting period. Our international family health insurance guide explains those rules; the calculator above adds a partner and up to four children.
What do 10 years of cover cost at today’s prices?
Ten years of cover from age 60 cost €96,600 in Zone 1 Comfort with a €1,200 deductible at today’s prices, excluding the annual repricing (AVB §5) and any surcharge. The table assumes ten full calendar years from 1 January of the year you reach a band’s first age: five years in that band and five in the next, with the step on 1 January of the year of the birthday.
Today’s prices; excludes annual repricing (AVB §5) and surcharges. Age steps on 1 January of the birthday year. Premiums only; deductible payments not included.

What this projection leaves out. It excludes the yearly premium review under AVB §5, which can raise or lower prices from 1 January of any year, and any surcharge from the health interview. It includes only the age-band steps, which happen on 1 January of the year your birthday falls in, not on the birthday itself. Joining mid-band shortens the time at the lower price. Treat the totals as a rough planning figure, not a quote: repricing can move them either way, and the deductible you actually pay comes on top.
When does your premium move to the next age band?
Your premium moves to the next age band on 1 January of the calendar year in which you reach that band’s first age. The insurer counts age as the current calendar year minus your year of birth, so the month of your birthday makes no difference to the price.

The same calendar-year rule decides the entry deadline: you count as 65 from 1 January of the year of your 65th birthday, so the latest start date is 1 December of the year you turn 64. The written terms (AVB) phrase the entry limit as “65th birthday not yet reached”, but the application form and FAQ use the calendar-year rule; plan with the stricter one. Our age-limit guide has a deadline calculator.
How do other insurers’ prices compare?
We have no verified price data for other insurers, so this index does not compare them. Other international health insurers set their own age bands, zones, deductibles and benefit limits. We hold partner data for Care Expat World alone, so this index contains no figures for any other plan, and a comparison would mean inventing numbers. If you compare quotes yourself, match the zone, the deductible, the annual and lifetime limits, the exit age and the rules on pre-existing conditions before you compare the monthly price. For public alternatives by destination, see healthcare systems by country; for budgets beyond insurance, see the cheapest countries to retire.
How was this price index built?
We transcribed the insurer’s own published premium table and checked every one of the 297 figures against the original document. The details:

What we did
Source
Care Concept AG, official English all-premiums PDF for Care Expat World (one page per zone), read on 27 October 2026 (Zone 1) and 28 October 2026 (Zones 2 and 3); age-band rule from the premium overview in the German consumer information pack
Transcription
All 297 prices entered once into our price registry; the CSV file and every table on this page are generated from that registry, so they cannot drift apart
Check
CSV compared value by value with the registry: 99 rows, no mismatch (28 October 2026)
Age rule
Age = calendar year minus year of birth (premium table footnote); a new band applies from 1 January of the year you reach it (consumer information pack)
Zones
Zone 1: worldwide abroad except the USA, China and Singapore. Zone 2: adds China and Singapore. Zone 3: adds the USA (insurance terms, AVB §6)
Not included
Individual surcharges after the health interview; future yearly repricing (AVB §5); payment fees
Our calculations
Percentages rounded to whole numbers; yearly amounts = 12 × monthly; projections at today’s prices only
Update policy
We refresh the index when the insurer publishes a new table and change the data date on this page and in the file name
The German terms are the legally binding text; English documents are offered as a service. Where documents conflict (the entry-age wording, for example), we say so on the page. We do not adjust, smooth or round the insurer’s prices.
Can I download and reuse the data?
Yes: the full table is a free CSV file, and you may quote it with a source. The file care-expat-world-premiums-2026.csv has one header row and 99 data rows with the columns zone, tier, band, deductible_0, deductible_1200 and deductible_3600; each price is a whole number of euros per month.
How to cite this data
“Expat Health Insurance Price Index 2026: Cost by Age and Zone”, Expat Health Guide, /expat-health-insurance-price-index/, premiums as published by the insurer and read on 27–28 October 2026, page updated 2026-10-28.
Which Care Expat World level fits you at 50 to 64?
The plan featured on this site. We earn a referral commission if you take it out, at no extra cost to you (how we are paid); this is information, not personal advice. Lowest prices (Basic, €3,600 deductible): €140 at 50–54, €325 at 60–64; €40 for ages 0–24.
Care Expat World Basic (hospital and doctor cover)
Lowest price, for people who mainly want protection against large medical bills.
Age 50-54€140–€290
Age 60-64€325–€460
Per month, Zone 1. Lower price with a €3,600 deductible, higher with none.
Hospital in a single or double room, chief physician
The cards above show Zone 1 prices. Home-country cover applies only to EU or Schengen home countries, so US and UK citizens are not covered on visits home. Already 65 or older in the year cover would start? This plan is closed to you; we are preparing a comparison of plans without an entry-age limit.
Why look at Care Expat World when you are 50 to 64?
What it gives you
A large German insurer behind it: the risk carrier is Barmenia Krankenversicherung AG, part of the BarmeniaGothaer group (premium income of about €9.27 billion expected for 2025, company announcement of 15 December 2025), supervised by Germany's financial regulator BaFin.
No annual maximum on total benefits, and no fixed end date or end age once you are in.
The insurer gives up its right to cancel in the normal way, so it cannot end your cover with ordinary notice, for example after large claims.
Free choice of doctor, with no network you must stay inside.
Pre-existing conditions are judged one by one in a video interview, with no blanket exclusion.
Check-ups, dental treatment and vaccinations are paid without touching your deductible (where your level covers them).
No fixed home address needed, and you can apply while already living abroad.
14 days to change your mind after you sign.
What to know first
Cover must start by 1 December of the year you turn 64.
Minimum term 12 months; cover always starts on the 1st of a month.
The interview can lead to a surcharge, an exclusion or a refusal.
10-month wait for pregnancy, dentures and orthodontics.
No home-country cover if your home country is outside the EU/Schengen area (for example the USA or the UK).
Not for people living in Germany, and no written confirmation yet that it meets any specific visa rule.
Frequently asked questions
How much does expat health insurance cost at age 60?
In the Care Expat World table, a person in the 60–64 band pays from €325 a month in Zone 1 (Basic, €3,600 deductible) to €800 (Premium, no deductible). With the USA included (Zone 3) the range is €960 to €1,840. Your band follows the calendar year: you count as 60 from 1 January of the year of your 60th birthday.
How much does international health insurance cost at age 50?
At 50–54, Zone 1 prices run from €140 to €510 a month, depending on level and deductible. Comfort with a €1,200 deductible costs €335.
How much does international health insurance cost at age 30?
At 30–34, Zone 1 prices run from €70 to €350 a month. The 35–39 band has exactly the same prices, so nothing changes at 35.
Is there an official average price for expat health insurance?
Not that we found. No regulator or statistics office we know of publishes a market average for international health insurance. This index is one insurer’s complete published table, so it shows real prices for one plan, not a market average.
Where can I download expat health insurance premiums as a spreadsheet?
Download the CSV file from this page (care-expat-world-premiums-2026.csv). It has 99 rows, one per zone, level and age band, and one column per deductible, so 297 prices in total. It opens in Excel, Google Sheets, Numbers and LibreOffice.
How often is this price index updated?
We refresh it when the insurer publishes a new premium table. The insurer reviews premiums every year and any change takes effect on 1 January, so the first check is due when the 2027 table appears. The data date is printed above every table.
Which age band has the biggest price jump?
For Comfort with a €1,200 deductible in Zone 1, the biggest step is from 50–54 to 55–59: +51%. Other levels and deductibles step differently; the jump table on this page lists all nine combinations.
Why do the 30–34 and 35–39 age bands cost the same?
The insurer’s table simply prints identical prices for the two bands in every zone, level and deductible. We copy the table as published and do not know the insurer’s reason.
How much more is cover including the USA at age 60 to 64?
At 60–64, Zone 3 (with the USA) costs 130% to 205% more than Zone 1, depending on level and deductible. Note for Americans: the USA is your home country, so the plan never covers you there, whatever zone you choose.
Is Zone 2 the same price for China and Singapore?
Yes. China and Singapore are both in Zone 2 and share one price table. The documents do not name Hong Kong, Macau or Taiwan, so ask Care Concept in writing which zone applies there.
How much does the €3,600 deductible save at 60 to 64?
In Zone 1 Basic at 60–64, choosing the €3,600 deductible instead of none saves €1,620 a year in standard premiums. Over a full calendar year, one person comes out ahead when the bills that fall under the deductible stay below that amount; above it they pay up to €1,980 more than without a deductible. A surcharge after the health interview or a yearly price review can change the saving.
When does the yearly premium saving cover the whole deductible?
When the standard premium saved over a full calendar year is at least as large as the deductible itself. In the 2026 table that holds for 17 of 198 deductible choices across all zones (11 in Zone 1, 16 with the €1,200 deductible); in 6 of them the saving only equals the deductible. Conditions: prices after the health interview can carry a surcharge, the yearly review can change the gap from 1 January, the first-year deductible is pro-rated by month (AVB §7 IV), and the documents don’t say whether a family pays it per person or per contract. This describes the price table; it is not a recommendation.
How much does expat health insurance cost for a couple aged 60 to 64?
Two people at 60–64 on Comfort with a €1,200 deductible in Zone 1 pay €1,310 a month, or €15,720 a year. Each person pays the price of their own age band; there is no couple discount in the table.
What do children pay on Care Expat World?
Children are priced in the 0–24 band until the year they turn 25. In Zone 1 that is €40 to €250 a month per child, depending on level and deductible.
In which year does my premium move to the next age band?
From 1 January of the calendar year in which you reach the band’s first age (25, 30, 35 and so on up to 70), not on your birthday. Someone born in October 1966 pays the 60–64 price from 1 January 2026.
Does the price index include surcharges after the health interview?
No. Every price is the standard premium before any individual surcharge. After the video health interview the insurer can add a surcharge, exclude a condition or decline the application.
Will the 2026 premiums still apply in 2027?
Not necessarily. The insurer compares claims and premiums every year and can adjust premiums from 1 January; after an increase you may leave within one month. Age-band steps come on top of that.
Can I quote these premium figures on my own website?
Yes, as facts with a source. Name the plan and the date, and cite this page by title and path as shown in the citation box. Please do not present the figures as current after the insurer has published a new table.
Who owns the copyright in the premium table?
The prices are the insurer’s; the table layout, analysis and CSV were compiled by Expat Health Guide. We do not attach a separate open licence to the insurer’s figures.
Why are the premiums in euros and not dollars or pounds?
The plan is a German group contract and premiums are set and collected in euros. Your bank or card provider converts them, so the dollar or pound amount moves with the exchange rate.
What is the last age band in which I can join?
The 60–64 band, and only if cover starts by 1 December of the year you turn 64. The 65–69 and 70+ prices in the table apply to people who joined earlier. Already 65 or older in the year cover would start? This plan is closed to you; we are preparing a comparison of plans without an entry-age limit.
Do I pay the full deductible in the first year?
No. If cover starts during the year, the deductible for that first calendar year is reduced pro rata in full months, starting with the month cover begins.
Do Americans and Britons pay the same premium in this index?
Yes. The table has no nationality factor: price depends on zone, level, deductible and age band. What differs is where you are covered, because the plan never covers you in your declared home country unless that country is in the EU or Schengen area.
How do I work out the premium for my own year of birth?
Use the calculator at the top of this page: pick the zone, level, deductible and cover start, enter your year of birth and, if you like, your partner’s and your children’s years of birth. It shows the monthly and yearly total and the date of your next age-band step.
